ABOUT PRESBYOPIA

What is presbyopia? 

Presbyopia is an age-related near vision problem that develops in people older than 40 years of age. The shape of the eye lenses may have a tendency to alter. The lens may change its shape by expanding when objects come near thanks to its special capability, thus enhancing vision quality. That is how near vision occurs. After the age of 40, the human eye gradually loses this capability.

Why does presbyopia develop?

When a distant object comes near,  the signal reaching the brain is assessed and transmitted to the eye. Here, the muscles in the ciliary body contract and relax, thereby tensing or relaxing the fibers attached to it. This movement of the fibers makes the lens get thinner or thicker and increases its refractive power. The eye loses this ability with aging. Although not proved, the most plausible theory for the age-related loss of this ability is the theory of cell aging. This theory claims that our eye cells lose this ability with aging, just like our hair whitens. Therefore, it is seen not as a pathological condition but as a physiological sign of aging. Presbyopia progresses over the years in everybody in a very standard manner. After 40 years of age, it increases in magnitude +1 every 10 years. At 50 years, it has increased +2, and +3 at 60 years. However, there is an interesting detail here. For example, if the eye is -2, +1 is added at the age of 40. Then you need -1 diopter glasses for near vision. When you become 50 and need -2 diopter glasses for far vision, you no longer need glasses for near vision. So those myopic patients who say “I have very good near vision” do so because two disorders overlap and alleviate the defect.

What are the symptoms of presbyopia?

Presbyopic patients have difficulty in focusing on objects nearer than 50 cm. So they can focus on objects by moving objects away from their eyes.

How is presbyopia treated?

There are many treatment methods used for this disorder around the world. These treatment methods have certain advantages and disadvantages. Intraocular multifocal lens implantation surgeries, which are recognized as the most reliable method for the treatment of presbyopia throughout the world, as well as presbylasik and corneal inlay (Kamra) operations are successfully performed at Dünyagöz Hospitals.

CORNEAL INLAY TREATMENT

The corneal inlay (Kamra) treatment is a reliable treatment method preferred by many ophthalmologists, which has been developed to treat age-related near vision problem.

The treatment consists of a micro-implant which is compatible with the eye’s structure and placed in the cornea, the eye’s outermost layer. It is lighter than a grain of salt and never noticed in the eye. Kamra inlay treatment can only be administered in operating rooms equipped with the IFS technology. IFS is the most advanced version of Intralase Femtosecond Laser.

Course of Treatment 

In Kamra Inlay treatment,  the operation is performed under topical anesthesia (eye drop) from beginning to end. A clean flap is formed in the cornea by IFS. If the patient is near-sighted, this problem is corrected first by laser treatment. Immediately afterwards, the Kamra implant is placed through the tunnel opened by IFS Laser and the operation ends. No pain is felt during the entire procedure. The treatment is completed in about 10 minutes. Stitches or bandages are not used after treatment. The patient can be discharged after a 15-minute rest following the operation and return to their social life the very next day.

The treatment corrects both near- and far-sightedness.

Unlike other presbyopia treatments, Kamra Inlay treatment doesn’t affect near vision negatively and corrects far vision problems in the same operation.

PRESBYLASIK SURGERY

Another method used as a solution to near vision disorders is laser treatment. Near vision disorders can be eliminated with a laser method called Presbylasik. In order to be able to undergo near vision laser treatment, your eye structure should be deemed suitable as determined by a series of examinations to be performed.

MULTIFOCAL INTRAOCULAR LENS TREATMENT

Intraocular Lenses

In this treatment method, the natural lens in the eye is removed and replaced with a lens with multifocal functionality. There are some rings in these lenses. Some of these rings provide far vision and some near vision. Thus, both distant and near objects are projected clearly on the eye’s yellow spot, which provides visual acuity. Patients can adapt to this very quickly and get rid of their glasses for near and far vision.

Patient selection is very important in this treatment. It should be considered carefully if near or far vision is more important for the patient. The patient’s pupillary size is another significant consideration in patient selection. Another advantage of the treatment is that it offers a simple solution for patients who need to undergo cataract surgery by eliminating cataract and solving the near- or far-sightedness problem in the same session.

First of all, the patient should have a detailed eye examination. Not every patient’s eye may be suitable for this type of surgery. The professional status, age, social activity, reading habits and intellectual level of cataract patients are also significant in patient selection. With correct patient selection, appropriate technique, experience of the physician and the institution where the operation takes place – and of course correct lens selection – high success rates are achieved in this type of surgery.
