Diabetes is a systemic and metabolic disease with high
blood sugar levels. Diabetes, which affects the whole body from head to
toe, especially affects our organs including small vascular networks. One
of them is the eye. Diabetes shows its most important effect on the eye by
affecting the small vessels in the nerve layer called retina, which disrupts
the nutrition of this layer. As a result, bleeding, deposits, fluid
accumulation, in other words edema, occurs in the retinal layer. This
picture is called diabetic retinopathy.
Retinal edema occurs in the visual center we call the
macula and gradually causes a decrease in vision. Sometimes it does not
give any symptoms without fluid accumulation and causes vision loss by
insidiously progressing and making sudden intraocular bleeding.
Diabetes patients should have an eye examination
immediately after the diagnosis. The diagnosis is made in the fundus
examination of the eye examination. After diagnosis, ocular tomography
(OCT) and eye angiography (FFA) are required to evaluate the degree of diabetic
retinopathy and treatment options.
The most important step in treatment; It is to solve
the problem such as high blood pressure and blood cholesterol together with
strict blood sugar control to prevent it from happening. If diabetic
retinopathy has developed, argon laser photocoagulation is performed according
to the degree after eye angiography and tomography. If there is fluid
accumulation (edema) in the macula (visual center), intraocular needle therapy
comes into question. Generally, a single session of laser or needle
therapy is not enough. In advanced cases, intraocular bleeding and bands
that occur with the operation we call vitrectomy can be removed.
When diabetes is diagnosed, the first eye examination
should be done. Afterwards, if there is no problem, an examination should
be made at least once a year. After the onset of diabetic retinopathy, the
follow-up period can be reduced by 1-6 months, depending on its severity.
