In the microinjection process, which is used extensively
in IVF applications, living sperm cells are enlarged up to 400 times. In
this magnification, the general morphological features (head structure, neck
structure and tail structure) of the sperm can be examined. However,
structures that should not be like defects and vacuoles (fluid-filled sacs) in
the head structure of the sperm cell cannot be examined.
With the method called IMSI, sperms can be enlarged up to 8000 times instead of
400 times by means of large magnification lenses and special optical systems to
distinguish the defective structures in the sperm head. In this way, the
selection of normal or nearly normal sperm is provided.
In studies conducted intensively since 2005, the vacuoles in the nucleus
containing the genetic material in the sperm head give clues that there may be
damage to the DNA structure. Damages in the sperm DNA structure can cause
problems such as fertilization failure, cessation of embryo development, poor
or slow embryo development and thus negatively affect the chance of pregnancy. It
is also very difficult for sperms with high DNA damage to obtain pregnancy
naturally except in vitro fertilization. Sperms with damaged DNA are
naturally eliminated in pregnancies with normal routes. Studies show that
pregnancy rates are higher in patients who have been applied IMSI compared to
the group not applied, but also show that post-pregnancy losses (clinical
abortion-abortion) are significantly reduced.
In men; Conditions such as advanced age, smoking, air pollution, prolonged
sexual abstinence, exposure of testicles to abnormally warm environment have
been observed to increase the damage to sperm DNA. If the sperm DNA damage
is less than 8%, the DNA of the eggs can repair the damaged DNA of the sperm
and a healthy child can be born.
