Radiotherapy is a treatment method of ionizing beam or atomic particles used in the
treatment of cancer or, rarely, non-cancer diseases.
Radiation shows biological and physical effects by ionizing the atoms and molecules it
collides with while passing through matter and biological environment. Ionizing
radiations are one of the cornerstones used in modern cancer treatment because
of their lethal effects on tumors and other cells or during their division.
In parallel with the development of computer technology, there have been developments in
the technology of external radiotherapy devices that generate ionizing
radiation used in radiotherapy planning and treatment. LINAC-based devices
have been developed. The common feature of these developed devices is that
they can perform IMRT and IGRT.
In line with these developments, our hospital has added the VARIAN TRILOGY DEVICE to the
hospital with the radidarc license with the latest technological features.
The TRILOGY DEVICE in Avrasya Hospital Oncology Department is a linear accelerator (LINAC)
device used in external (external) radiotherapy.
It produces 6MV and 16MV photon (x-ray) and 4,6,9,12,16,20 MeV electron energy.
It is the advanced form of 3D (3-dimensional) conformal treatment that we have used until
now in the radiotherapy device. With the radiotherapy device, instead of
sending a single beam of rays to the human body, treatment is performed from
areas where density differences are created by dividing the radiation into
thousands of rays. Thus, while the healthy tissues and organs are
protected in the best way, it is ensured that the highest dose of radiotherapy
is given to the diseased tissue.
Thanks to the IMRT technique, the desired high dose is
given to the tumor, while the dose is kept at a low level in the normal tissues
adjacent to the tumor tissue with the sudden reduction of the dose in the
junction area of the
tumor and intact tissue and the use of multiple areas.
Calculations are made with the Treatment Planning
Computer System to determine the dose of radiotherapy to be applied to the area
to be treated. Planning time is long as it requires more precise
calculations.
The device rotates around the patient in one or more circles. In this technique,
while tumor tissue is irradiated with a more sensitive and high dose,
neighboring intact tissues are protected from radiation at a very high rate.
With the RapidArc technique, the daily treatment time of patients is relatively shorter
than other devices that do the same job. In addition, RapidArc has the
advantage of using less monitor units (radiotherapy dose units) compared to
other devices that apply intensity-adjusted radiotherapy technique.
Thanks to this feature of the Trilogy device, it can irradiate very small tumors with a
size of a few centimeters. Thus, a very high dose of concentrated ionized
radiation is delivered to the tumor. Damage to healthy tissues from radiation
is minimized by providing a very rapid dose reduction in the normal tissue
around the tumor.
When stereotactic radiosurgery feature is used, the treatment of the patient is
completed in a few sessions. However, this feature of the device should be
applied to properly selected patient groups.
Until recently, a wide safety margin was used during the treatment because the tumor
and the irradiated area were not completely stable during radiotherapy and
could move during irradiation. Unfortunately, healthier tissue would be
exposed to radiation. Side effects due to radiotherapy were higher. Given
the small margin of safety, there was a risk that the tumor remained outside
the area during treatment. In line with the technological developments in
radiotherapy devices, we take the image of the treatment area to check whether
the patients are irradiated to the right place before, after the treatment and
most importantly during the treatment. This is called the device’s IGRT
feature. In this way, if there are differences in the images we receive
from the area we irradiated, we make the necessary corrections and make our
treatment more accurate.
