Sperm search techniques by surgical method – tese, pesa, mesa

In cases where there is no sperm in the semen or the
quality or vitality of the sperm in the semen is very low, sperm can be
surgically obtained from the ducts, epididymis or testis. These procedures
are performed under local and general anesthesia so that the patient does not
feel pain. The patient is discharged on the same day without the need for
hospitalization.

 In cases where sperm production occurs in the testicles

called obsrtuctive azoospermia , but the sperm does not pass into the
semen due to the obstruction in the ducts, PESA (obtaining sperm from the
epididymis with a needle) or TESA (sperm extraction from the testicles) is
performed.

Non obstructive azoospermiaIn cases where sperm cannot be seen in the semen

due to the sperm production problem in the testicles called testicles, sperm
can be obtained by TESE (searching for sperm by taking biopsies from the
testis) or micro TESE (by scanning the areas where sperm can be produced in the
testicles under microscope. The first pregnancy was achieved in 1995.

After these procedures, there may be mild pain and sensitivity, but generally
regresses in 24-48 hours. There is a risk of infection and bleeding after the
procedure, therefore it is recommended to wear tight underwear after the
procedure.

Surgically obtained sperms are injected into the mature egg using the ICSI
technique. Excess sperm or testicular tissue obtained surgically can be
stored by freezing. Thus, there is no need for repeated surgical
procedures. When sperm cannot be found, the egg collection process is
canceled. Within the current information, there is nothing left to do when
no sperm can be found with the micro TESE process, which is the most advanced
technique.
